Why is this joint still sore? You may have done more walking, lifting, or yard work than it could handle. Old wear or an earlier injury can also make a busy day hurt. The ache may begin during use, later that evening, or the next morning. When it starts matters.

What to notice about the ache

Soreness in the first few steps may go with stiffness after rest, while an ache that builds all day may mean repeated use keeps stirring the joint hour after hour. Night pain matters because it can ruin sleep and deserves a closer look. None of these signs proves the cause by itself.

Notice swelling, weakness, catching, or lost motion. I’d also write down how the joint feels the next morning. Think back to extra stairs, lifting, golf, or a longer outing. If the ache began with a snap, hard blow, or sudden weakness, don't treat it as simple overuse.

What to try while it settles

Ease back on the task that brings on the ache. You can still move in ways that feel comfortable. Try shorter walks, lighter jobs, or fewer trips up the stairs. Gentle motion may loosen stiffness. If you hurt much more that evening or the next morning, do less.

Tendinopathy is described in the Nature Reviews Disease Primers review as a complex, multifaceted tendon pathology - disorganised collagen fibres, increased microvasculature and sensory nerve ingrowth, dysregulated matrix homeostasis, increased immune cells and inflammatory mediators, and enhanced cell apoptosis - most commonly affecting the rotator cuff, the medial and lateral elbow epicondyles, the patellar tendon, the gluteal tendons and the Achilles. The authors state plainly that management consists of exercise and loading programmes, therapeutic modalities and surgery, and that their effectiveness 'remains ambiguous'.

  2. The continuum model argues that describing tendinopathy simply as degenerative or as failed healing does not explain why people of different ages, with tendons under different loads, present with such varying pain, irritability and function - or why some tendons respond to simple treatment and some resist everything. It proposes a continuum of pathology instead, which allows treatments to be placed rationally along it.
